Users - What are the situations that bring transaction to rollback

what are the circumsatances that may a transaction rollback.

I get the following error :
[TransactionUtil.begin] active transaction marked for rollback in place, so no transaction begun; this stack trace shows when the exception began:

How does this happen so suddenly?

short answer, when a Database rejects or can not complete a SQL
statement sent to it.
