docbook HTML rendering

Hi all,

as you may have see, I have corrected the HTML rendering for caution,
etc... in docbook. It is now using the right syntax, as told by the DTD.

I've also added some CSS styles, which are just here for decoration...
If anyone could take a look and create the right style for each theme,
this would be a nice addition.
