I don't know what exactly is going on on your side but see what I got using patch with the 2nd patch.log attached.
I noticed you have a mix of Win and Unix EOLs in your file. By forcing them to CR+LF and using Subclipse I was able to create the newly attached patch where all your changes are applied but this hunk
