For some reason the BOM creation form does not include all of the fields that exist on the edit page.  The thruDate, sequenceNum, reason, instruction, scrap factor, formula, and routingWorkEffortId fields are missing.  I think the form should be changed so that these fields can be set when BOMs are initially created.
