COD Surcharge

Most of the time shippers charge for a COD payment. AFAIK, apart from UPS (in shipment.properties) there are no means to deal with that from a standard estimate.
I'd like to add a COD surchage option in shipping estimate definition. It would be a flat charge added to flat charges group.
If an estimate containing a COD surcharge is chosen its value would be concatenated to the COD payment label (not included in estimate total) to let know the user and used in the cost calculation if the user choice COD payment.
