Getting error while updating item from order view page.

Getting error "The changed return is can not be greater then the cash given". Seems that, it occurs due to duplication of default promotion associated with Order.
May be there is wrong with recalculating cart values.

Here is a patch to fix this issue.

1) It prevents to apply duplicate promotion on order .
2) Save additional adjustments of order.
